Uninstall OMG Music Toolbar

I’m running vista 32 bit with windows internet explorer. When i click “View”and then toolbars, i notice that I have a toolbar called OMG Music Toolbar which I do not want and even don’t know how it got there. I have tried everything to delete it but it is still there. Also I find that my homepage has been changed to a website I am not familiar with. It shows OMG Search box in the middle of the page and displays various ads below. How do I get rid of it?
